• Coordinate care between members, caregivers, and healthcare providers to ensure person-centered care.
• Monitor member status, update care plans, and collaborate with providers to meet member goals.
• Provide resource support for local services, such as employment, housing, and independent living.
• Facilitate care management and collaborate with providers to ensure timely access to needed care.
• Collect and maintain member information and care management activities.
• Provide education to members and caregivers on disease processes, care options, and healthcare benefits.Requirements:
  • Compact Registered Nurse license.
