Job Description:

Transform the health of communities for 28 million members as a registered nurse with Centene.Coordinate care between members, caregivers, and healthcare providers to deliver person-centered care.

Follow up with members to ensure they receive adequate care and services.
Collaborate with providers to revise care plans and meet member goals.
Provide resource support for local services like employment and housing.
Facilitate care management and ensure timely access to care.
Collect and maintain member information for compliance with regulators.
Provide education to members on disease processes and care options.
